SKIMS Patient Card Sample

Follow these simple steps to book:

  1. Identify the Patient ID on your card (highlighted in green on the sample image).
  2. Locate your First Name on the card (highlighted in red on the sample image).
  3. Select your clinical department or clinic name from the dropdown list.
  4. Click on Book Appointment to proceed to slot calendar page.
